ABSTRACT

O leite é a melhor fonte de cálcio na alimentação, seu consumo é importante principalmente nas fases pré-escolares e escolares, sendo necessária a ingestão de 3 copos (600 mL) de leite por dia para suprir a quantidade de cálcio. A avaliação da ingestão de leite é importante para permitir o cálculo da ingesta de cálcio, vitaminas e minerais e possíveis resíduos e contaminantes como agrotóxicos e micotoxinas, entre outros. O objetivo deste estudo foi avaliar o consumo de leite e produtos relacionados, em crianças na faixa etária de quatro meses a três anos, de uma instituição pública com fins filantrópicos localizada no município do Rio de Janeiro. O projeto foi aprovado pelo comitê de ética da Fundação Oswaldo Cruz (FIOCRUZ) sob o número 641/2011. Na creche a avaliação do consumo foi realizada contabilizando a quantidade de leite preparada menos a consumida e o resto deixado pelas crianças. A avaliação do consumo na residência foi realizada através de entrevista com os responsáveis e preenchimento do questionário estruturado. Foi possível identificar: o consumo de leite na residência, o tipo de leite consumido (UHT, pasteurizado, pó) e a quantidade de leite em pó utilizada no preparo do produto. Pode-se concluir que na residência 88% das crianças, consomem leite, com regularidade, 9% o fazem esporadicamente e somente 3% não consomem o produto por motivos alérgicos ao alimento, consumindo fórmulas especiais e produtos à base de soja. Verificou-se que 59% dos responsáveis que utilizam o leite em pó, preparavam o produto em desacordo com o valor recomendado pelo fabricante (VR=0,13 g.mL-1). Dentre esses 34% prepararam o leite com menor quantidade de pó adicionada à quantidade de água recomendada e 25% o fazem usando excesso de leite em pó a água adicionada no preparo. Na creche, foi possível observar o baixo consumo de leite infantil na instituição.

ABSTRACT

BACKGROUND: Based on personal observations of dermatologists, nutritionists and patients, a new problem emerges in the gyms: development and exacerbation of acne in users of whey protein, which is a protein derived from cow's milk. Whey Protein extract contains growth factors that may be related to acne. Its purity and composition are not fully known and there is no scientific research on its potential to cause acne. OBJECTIVES: Assess the relationship between the use of proteincalorie supplements and onset or exacerbation of acne vulgaris in young adults in the city of João Pessoa. METHODS: Descriptive observational study conducted in gyms and in a dermatology clinic. Each participant was examined for acne on three occasions and followed for 60 days. RESULTS: 30 patients were examined. Onset or exacerbation of acneiform lesions were observed (p <0.0005). The effect was more prominent in females and in individuals without current acne and no family history of acne. CONCLUSION: This study showed the onset of acne with the progressive use of protein-calorie supplementation, in a two-month period. It is suggested that this type of supplementation be discouraged. .


FUNDAMENTOS: Na observação pessoal de dermatologistas, nutricionistas e dos próprios pacientes, um novo problema emerge nas academias de ginástica: desenvolvimento e exacerbação da acne em usuários de Whey Protein, proteínas derivadas do leite de vaca. Extratos de Whey Protein contêm fatores de crescimento que podem estar relacionados com a acne. Sua pureza e composição não são completamente conhecidas e não há investigações científicas sobre seu potencial causador de acne. OBJETIVO: Avaliar a relação entre o uso de suplementos protéico-calóricos e o aparecimento ou agravamento da acne vulgar em adultos jovens em João Pessoa - PB. MÉTODOS: Estudo observacional descritivo conduzido em academias de ginástica e em consultório de dermatologia. Cada participante foi examinado para acne em três ocasiões e acompanhado por 60 dias. RESULTADOS: Foram examinados 30 pacientes. Aparecimento ou exacerbação de lesões acneiformes foram observados (p<0,0005), sendo o efeito mais proeminente no sexo feminino e em indivíduos sem acne atual e sem história familiar de acne. CONCLUSÃO: Este estudo demonstrou o aparecimento de lesões acneiformes com o uso progressivo de suplementos proteico calóricos, ao longo de dois meses. Sugere-se que o uso deste tipo de suplemento seja desestimulado. .

ABSTRACT

Introducción: La calidad de vida y estado nutricional del lactante con alergia a la proteína de leche de vaca (APLV) pueden comprometerse sin un adecuado diagnóstico y tratamiento oportuno. Objetivo: Describir síntomas digestivos, antecedentes familiares y perinatales, estado nutricional y respuesta de lactantes < 12 meses con APLV entre junio de 2007 y agosto de 2011. Pacientes y Método: Estudio retrospectivo, descriptivo en lactantes < 12 meses de edad que consultan por sospecha de APVL. Se investigaron antecedentes familiares, perinatales y características clínicas. Se excluyó a los casos sin prueba de alimentación o provocación abierta, o cuyo motivo de consulta correspondió a otra patología. Se usó el programa STATA especial edición 11,1 para el análisis estadístico. Se consideró estadístico y significativo un p < 0,05. Resultados: Se diagnosticaron 40 lactantes (57,5 por ciento varones), edad 3,0 +/- 0,4 meses, clínica de 2 +/- 0,3 meses de: vómito/regurgitación: 20, diarrea sanguinolenta: 8, falla en el crecimiento: 4, cólico/irritabilidad: 4, deposición normal con sangre en 3. Los antecedentes familiares y perinatales fueron ingesta de biberón en la 1ª semana de vida: 35, madre > 30 años: 31 casos, atopia en 2 familiares (padres/hermanos): 28, parto por cesárea: 27. Al seguimiento de 14 +/- 1,4 meses, 19 respondieron a fórmula ampliamente hidrolizada +/- seno; 19 a fórmula de aminoácidos +/- seno y un paciente lactancia exclusiva. El Z-score de P/E:-1,05 +/- 1,28; T/E: -0,96 +/- 1,60; IMC: -0,61 +/- 1,03 en primera consulta, mejoró al año: P/E: -0,15 +/- 1,24; T/E: -0,29 +/- 1,14 e IMC: -0,04 +/- 1,20 (p < 0,05), independiente de la fórmula (p = NS). Veintitres toleraron leche entera al año. Conclusiones: En este estudio los síntomas predominantes al inicio de los pacientes con APLV fueron vómitos, regurgitación y diarrea sanguinolenta. Introduction: Quality of life and nutritional status of infants with cow's milk protein allergy (CMA) can be compromised without a proper diagnosis and treatment. Objective: To describe digestive symptoms, family and perinatal history and nutritional status of < 12 month infants with CMA between June 2007 and August 2011. Patients and Method: A retrospective and descriptive study in < 12 month old infants suspecting CMA was performed. Family history, perinatal and clinical characteristics were studied. Cases without food test or open challenge test, or whose reason for consultation corresponded to other pathology were excluded. The program STATA 11.1 was used for statistical analysis and p < 0.05 was considered statistically significant. Results: 40 infants (57.5 percent boys), age 3 +/- 0.4 months and history of 2 +/- 0.3 months presenting the following: 20 of them (50 percent, vomiting/regurgitation; 8 (20 percent, bloody diarrhea; 4 (10 percent) failure to thrive; 4 (10 percent), colic/irritability and bloody normal deposition was present in 3 individuals (7.5 percent). Family and perinatal history: 35 (87.5 percent were bottle-fed during the first week of life; 31 (77.5 percent) had > 30 year old mothers; 28 (70 percent described 2 family members (parents/siblings) with atopy; 27 (67.5 percent were delivered via cesarean. At the 14 +/- 1.4-month follow-up, 19 (47.5 percent responded to extensively hydrolyzed formula +/- breastfeeding; 19 to amino acid formula +/- breastfeeding and one patient to breastfeeding exclusively. The Z -score for P/E was: -1.05 +/- 1.28; T/E: -0.96 +/- 1.60; BMI: -0.61 +/- 1.03 in the first consultation; after a year, scores improved: P/E: -0.15 +/- 1.24; T/E: -0.29 +/- 1.14 and BMI: -0.04 +/- 1.20 (p < 0.05), independent from formula (p = NS). 23 (74 percent) tolerated whole milk when reached a year old. Skeletal muscle is the major deposit of protein molecules. As for any cell or tissue, total muscle protein reflects a dynamic turnover between net protein synthesis and degradation. Noninvasive and invasive techniques have been applied to determine amino acid catabolism and muscle protein building at rest, during exercise and during the recovery period after a single experiment or training sessions. Stable isotopic tracers (13C-lysine, 15N-glycine, ²H5-phenylalanine) and arteriovenous differences have been used in studies of skeletal muscle and collagen tissues under resting and exercise conditions. There are different fractional synthesis rates in skeletal muscle and tendon tissues, but there is no major difference between collagen and myofibrillar protein synthesis. Strenuous exercise provokes increased proteolysis and decreased protein synthesis, the opposite occurring during the recovery period. Individuals who exercise respond differently when resistance and endurance types of contractions are compared. Endurance exercise induces a greater oxidative capacity (enzymes) compared to resistance exercise, which induces fiber hypertrophy (myofibrils). Nitrogen balance (difference between protein intake and protein degradation) for athletes is usually balanced when the intake of protein reaches 1.2 g·kg-1·day-1 compared to 0.8 g·kg-1·day-1 in resting individuals. Muscular activities promote a cascade of signals leading to the stimulation of eukaryotic initiation of myofibrillar protein synthesis. As suggested in several publications, a bolus of 15-20 g protein (from skimmed milk or whey proteins) and carbohydrate (± 30 g maltodextrine) drinks is needed immediately after stopping exercise to stimulate muscle protein and tendon collagen turnover within 1 h.

ABSTRACT

Las reacciones alérgicas incluyen un amplio espectro de reacciones clínicas. Con una incidencia del 15–30% de las enfermedades alérgicas y afecta a un 8% de los niños y específicamente a la proteína de la leche de vaca entre un 5–15% de los niños. Pueden manifestarse como intolerancia o alergia dando manifestaciones respiratorias, cutáneas o gastrointestinales que pueden ser inducidos por cantidades minimas de la ingesta deproteína y cuyo tratamiento básico es la exclusión de la proteína de leche sobre todo en los primeros años de vida.


Allergic reactions include a wide spectrum of clinical reactions. With a 15-30% incidence of llergicdiseases, affecting 8% of children and specifically to the protein in cow's milk 5-15% of children hey can manifest as intolerance or allergy giving respiratory manifestations, gastrointestinal or skin can be induced by minimal amounts of protein intake and whose basic treatment is the exclusión of milk protein specially inthe early years of life.

OBJETIVO: Verificar a prevalência de alergia à proteína do leite de vaca em crianças com sintomas atribuídos à ingestão do leite de vaca. MÉTODOS: Foram estudadas 65 crianças com sintomas atribuídos à ingestão do leite de vaca. A definição diagnóstica ocorreu após teste de desencadeamento alimentar oral aberto, realizado no mínimo 15 dias após dieta de exclusão e ausência de sintomas, com período de observação de até 4 semanas após o teste. Considerou-se caso (alergia à proteína do leite de vaca positiva; n = 35) criança com reaparecimento do sintoma que motivou a realização do teste, e comparação (alergia à proteína do leite de vaca negativa; n = 30) aquela sem sintomas após o período de observação do teste. RESULTADOS: A mediana de idade foi 5 meses (P 25-75 por cento 2-9 meses) no grupo caso e 7 meses (P 25-75 por cento 4-11 meses) no grupo comparação (p = 0,05). O teste não confirmou alergia à proteína do leite de vaca em 46,8 por cento dos pacientes com sintomas atribuídos à ingestão de leite de vaca. Reação tardia ocorreu em 77,1 por cento (27/35) dos casos com teste positivo, sendo 18/27 na primeira, 3/27 na segunda e 6/27 na terceira semana de observação. Encontrou-se associação estatística significante entre manifestações cutâneas e teste positivo (p = 0,04), mas não com sintomas digestivos e respiratórios. CONCLUSÃO: Os resultados corroboram a necessidade do teste de desencadeamento alimentar oral para determinar os pacientes que realmente têm alergia à proteína do leite de vaca e se beneficiarão com dieta de exclusão de leite de vaca.


OBJECTIVE: To determine the prevalence of cow's milk protein allergy in children with symptoms attributed to cow's milk intake. METHODS: Sixty-five children with symptoms attributed to cow's milk intake were studied. Diagnosis was established after an open oral food challenge test carried out at least 15 days after an elimination diet and absence of symptoms, with a follow-up period of up to 4 weeks after the test. The children who remained asymptomatic after this period were considered negative for cow's milk protein allergy (n = 30), while those whose symptoms reappeared were considered positive (n = 35). RESULTS: The median age was 5 months (P 25-75 percent 2-9 months) in the case group and 7 months (P 25-75 percent 4-11 months) in the comparison group (p = 0.05). The test did not confirm cow's milk protein allergy in 46.8 percent of the patients with symptoms attributed to cow's milk intake. A delayed reaction occurred in 77.1 percent (27/35) of the cases testing positive, 18/27 in the first week, 3/27 in the second week, and 6/27 in the third week of follow-up. A statistically significant association was found between cutaneous manifestations and positive test result (p = 0.04). However, there was no association with digestive and respiratory symptoms. CONCLUSION: Our results confirm the need of an oral food challenge test to determine which patients really have cow's milk protein allergy and may therefore benefit from a diet free of cow's milk.

ABSTRACT

OBJECTIVE: We evaluated the efficacy of oral supplementation with milk whey proteins and modified starch (70 percentWPI:30 percentMS), on nutritional and functional parameters of patients with ALS. METHOD: A prospective randomized double-blind study was performed with 16 ALS patients, divided in two groups, the treatment group received (70 percentWPI:30 percentMS) and the control group received (maltodextrin). They underwent prospective nutritional and functional assessment for 4 months. RESULTS: Patients in the treatment group presented weight gain, increased body mass index (BMI), increased arm muscle area and circumference, higher albumin, white blood cell and total lymphocyte counts, and reduced creatine-kinase, aspartate aminotransferase and alanine aminotransferase. In the control group, biochemical parameters did not change, but weight and BMI declined. CONCLUSION: Our results indicate that the agglomerate 70 percentWPI:30 percentMS may be useful in the nutritional therapy of patients with ALS.


OBJETIVO: Avaliar a eficácia da suplementação nutricional oral com proteínas do soro do leite e amido modificado (70 por centoWPI:30 por centoMS), nos parâmetros nutricionais e funcionais de pacientes com esclerose lateral amiotrófica (ELA). MÉTODO: Foi realizado estudo randomizado duplo-cego, com 16 pacientes com ELA, divididos em dois grupos, um que recebeu 70 por centoWPI:30 por centoMS e um controle que recebeu maltodextrina. Os pacientes foram submetidos a avaliação nutricional e funcional durante quatro meses. RESULTADOS: Nos pacientes que receberam o suplemento 70 por centoWPI:30 por centoMS, foi observado ganho de peso, aumento na contagem de linfócitos e redução de creatina kinase, aspartato aminotransferase and alanina aminotransferase. No grupo controle, os parâmetros bioquímicos não sofreram modificações; no entanto, peso e índice de massa corporal diminuíram. CONCLUSÃO: Nossos resultados indicam que o aglomerado 70 por centoWPI:30 por centoMS pode ser útil na terapia de pacientes com ALS.

ABSTRACT

PURPOSE: To evaluate the effects of glutamine (L-Gln) or whey-protein supplementation on CD4+ and CD8+ lymphocytes in alloxan-induced diabetic rats. METHODS: Thirty-two healthy male Wistar rats were used in the experiment. Eight rats served as baseline controls (G-1). The remaining 24 animals received alloxan 150mg/Kg intraperitonially dissolved in buffer solution and were equally distributed in 3 subgroups, upon induction of diabetes mellitus, and treated as follows: (G2): saline, 2.0ml; (G3): glutamine solution (0.7g/kg), 2.0 ml; and (G4): whey-protein (WPS) solution (0.7g/kg), 2.0 ml. All solutions were administered by daily 7:00 AM gavages during 30 days. Next, arterial blood samples (3.0 ml) were collected from anesthetized rats for CD4+ and CD8+ lymphocyte count through flow cytometry technology. RESULTS: CD4+ and CD8+ counts decreased significantly in all groups compared with baseline values (G1). G2 rats CD4+/CD8+ ratio decreased significantly compared with G1. CD4+/CD8+ ratio increased significantly (>260 percent) in L-Gln treated group (G3) compared with saline-treated rats (G2). There were no statistical differences in lymphocyte counts (CD4+ and CD8+) between L-Gln (G3) and saline-treated (G2) groups. There was a significant reduction in CD8+ cell count compared with CD4+ cell count in L-Gln treated rats (G3). CONCLUSION: The offer of L-Gln to experimental diabetic rats enhances the immunologic response to infection.


OBJETIVO: Avaliar os efeitos da suplementação de glutamina ou proteína do soro de leite ( PSL) sobre os linfócitos CD4+ e CD8+ em ratos diabéticos aloxano induzidos. MÉTODOS: Trinta e dois ratos Wistar machos, saudáveis, foram utilizados no estudo. Oito ratos foram usados como controles basais (G1). Os 24 animais remanescentes foram equitativamente distribuídos em 3 subgrupos, após indução do diabetes mellitus por injeção intraperitonial de aloxano (150mg/Kg) e tratados como se segue: (G2): salina; (G3): 2,0 ml de solução de glutamina (0,75g/Kg);(G4): PSL, (0,7g/Kg), 2,0ml. Todas as soluções foram administradas por gavagem, diariamente, cada 7:00 h, durante 30 dias. Após esse período, foram coletadas amostradas de sangue arterial para contagem de linfócitos CD4+ e CD8+ por citometria de fluxo. RESULTADOS: A população de linfócitos CD4+ e CD8+ diminuiu significantemente em todos os grupos em comparação aos valores encontrados no grupo G1. A razão CD4+/CD8+ foi significantemente maior (>260 por cento) nos ratos tratados com L-Gln que nos ratos tratados com salina (G2). Não se observaram diferenças significantes na população de linfócitos CD4+ e CD8+ entre os grupos G3 e G2. Houve redução significante do número de células CD8+ quando comparado ao número de células CD4+ nos ratos tratados com L-Gln (G3). CONCLUSÃO: A oferta de L-Gln em ratos diabéticos aloxano-induzidos melhora a resposta imunológica à infecção.

The present study evaluated the acute effect of the intraperitoneal (ip) administration of a whey protein hydrolysate (WPH) on systolic arterial blood pressure (SBP) and renal sodium handling by conscious spontaneously hypertensive rats (SHR). The ip administration of WPH in a volume of 1 ml dose-dependently lowered the SBP in SHR 2 h after administration at doses of 0.5 g/kg (0.15 M NaCl: 188.5 ± 9.3 mmHg vs WPH: 176.6 ± 4.9 mmHg, N = 8, P = 0.001) and 1.0 g/kg (0.15 M NaCl: 188.5 ± 9.3 mmHg vs WPH: 163.8 ± 5.9 mmHg, N = 8, P = 0.0018). Creatinine clearance decreased significantly (P = 0.0084) in the WPH-treated group (326 ± 67 æL min-1 100 g body weight-1) compared to 0.15 M NaCl-treated (890 ± 26 æL min-1 100 g body weight-1) and captopril-treated (903 ± 72 æL min-1 100 g body weight-1) rats. The ip administration of 1.0 g WPH/kg also decreased fractional sodium excretion to 0.021 ± 0.019 percent compared to 0.126 ± 0.041 and 0.66 ± 0.015 percent in 0.15 M NaCl and captopril-treated rats, respectively (P = 0.033). Similarly, the fractional potassium excretion in WPH-treated rats (0.25 ± 0.05 percent) was significantly lower (P = 0.0063) than in control (0.91 ± 0.15 percent) and captopril-treated rats (1.24 ± 0.30 percent), respectively. The present study shows a decreased SBP in SHR after the administration of WPH associated with a rise in tubule sodium reabsorption despite an angiotensin I-converting enzyme (ACE)-inhibiting in vitro activity (IC50 = 0.68 mg/mL). The present findings suggest a pathway involving ACE inhibition but measurements of plasma ACE activity and angiotensin II levels are needed to support this suggestion.

ABSTRACT

A three months longitudinal followup of SGA term babies was done for their response to feeding of high protein milk (3.1 g/100 ml) and they were compared with their breast fed counterparts. The formula fed infants had no advantage over the breast fed when weight was compared. However, blood urea and serum creatinine levels were higher in the formula fed infants, than the breast fed ones. The serum valine concentrations, indicative of protein nutritional status, increased with increasing postnatal age in all the infants. Formula fed had higher serum valine than breast fed ones. Serum phenylalanine and serum tyrosine levels, which may hamper CNS development, were higher in the formula fed infants than the breast fed ones.
